At the Accident Scene

First Aid Only

If the injury requires first aid only:

  1. Instruct a qualified person to administer appropriate first aid immediately.
  2. Complete and submit the Report of Injury.
  3. Determine the cause of the accident and obtain names of all witnesses.
  4. Think about and apply corrective measures to prevent future recurrences of the accident.

Note: If you have any questions or doubts about the seriousness of the injury, always treat it as one requiring a physician's treatment.

Physician Treatment Required

If the injury requires physician treatment:

  1. Seek prompt medical attention by taking the injured employee to a physician or to the nearest emergency facility if warranted.
  2. Complete and submit the Report of Injury.
  3. Determine the cause of the accident and obtain names of all witnesses.
  4. Think about and apply corrective measures to prevent future recurrences of the accident.
  5. If a serious injury occurs and equipment is involved, do not move the equipment if it can be left at the scene of the accident. Additional investigation may be necessary. Take a picture of the equipment. Get the brand and model of the equipment, serial number, and any maintenance records. Contact the Clearpath Specialty Claims Department for further instructions.

Policies with Effective Date of 12/31/2023 and Prior

Policyholders should file a Report of Injury within 3 days of your awareness of the incident. The report is an alert to Clearpath Specialty that a claim needs to be investigated. Submitting the report is not binding.

Your Responsibilities When a Loss Occurs

  • Give prompt notice of a loss to Clearpath Specialty or your agent. REPORT AN INJURY
  • Notify the police if a law may have been broken.
  • Provide us with records and documents we request during a claim investigation and permit us to make copies.
  • If requested, submit to an examination under oath and/or submit a sworn statement of loss that sets forth your knowledge of the loss details.
  • Cooperate with us in the investigation or settlement of the claim.
