1. What Information We Collect.

To access our web site as a “Employer” or an “Agent”, and use certain Employer services or Agent services , we collect from Employers and Agents such Employers' or Agents’ name, address, email address, contact person name and telephone number. In addition, we collect the following “nonpublic personal information” about Employers, Agents or other users from the following sources:

  • Information you submit online, such as injury or illness reports, claim reports and other information submitted through this web site;
  • Information we receive from you when you request a quote for coverage online or when you submit applications or other similar information to us online;
  • Information about your transactions with us through this web site, including coverage purchased, claims reported, videos acquired or rented and other similar information; and
  • Information we receive from your computer when you visit our web site that is unique to Internet transactions, such as your Internet email address and operating system and statistical information on web site traffic and usage patterns. We may also collect and track this information on a group basis to help us identify the services that are most useful to Employers and Agents and to help us improve the site and our own marketing efforts.
  • “ Nonpublic personal information” is nonpublic information about you that we obtain through this web site in connection with providing our services to you, including issuing workers’ compensation insurance coverage, processing your workers’ compensation claims, among other services that we provide.

Our Use of Cookies.
Like many web sites on the Internet, our web site uses cookies. Cookies are small strings of data placed on a user’s personal computer hard drive during the exchange of data that happens when your browser points to our web site. Cookies enable a web site to identify a user's browser as a previous visitor by means of a unique string of numbers assigned on a previous visit. We do not disclose any nonpublic personal information about Employers, Agents, claimants or former claimants to anyone except as required or permitted by law and to enable us to provide services to Employers.   • An Employer's agent or broker.
  • Parties who perform a business or insurance function for us, including underwriting, claims administration or adjusting, claims investigation, loss control and computer systems companies.
  • Other insurance companies as authorized by you.
  • Insurance regulatory agencies or statistical compilation or reporting agencies.
  • Government entities or other authorized persons in response to subpoenas or court orders, as required by law.

We reserve the right to disclose personal data in special cases when we believe that disclosing information is necessary to identify, contact or bring legal action against someone who may be causing injury to or interference with our web site, other users of our web site, anyone else who could be harmed by such activities, or where we are required to do so by court order or applicable law. We reserve the right to disclose personal data when we believe the law requires it. 3. Independent Sales Agents or Brokers.

Workers’ compensation insurance coverage has been placed with us through an independent agent or broker (“Agent”). An Agent may have independently gathered information about the Employer. An Employer's Agent is responsible for the use and protection of Employer information that he or she collects. Employers who have questions about how an Agent uses or discloses Employer information should contact such Agent directly.

Changes in Privacy Policy

We reserve the right to change this privacy policy going forward. Changes will be posted on our web site, and we ask you to review the privacy policy anew when you see a notice indicating that the policy has changed. Your use of this web site constitutes your agreement to this privacy policy. If you do not agree to this privacy policy, then you should immediately discontinue use of this web site.
